 The levels of competence:
 Before answering the questionnaire, please refer to the levels of competence outlined here:

Level 1 Management and dispensing of devices that correct defects of the visual system
Level 2 Level 1, plus investigation, examination, measurement, diagnosis and correction/management of defects of the visual system
Level 3 Levels 1 and 2, plus investigation, examination and evaluation of the eye and adnexa and associated systemic factors, to detect, diagnose and manage disease
Level 4 Levels 1, 2, and 3, plus use of pharmaceutical agents and other procedures to manage ocular conditions/disease
